What we do
Our Outreach Services provide individuals with private tenancies access to the same high-quality care and support received by those who live within our Supported Living properties. These services can be accessed via social workers or self referral.
All support is built around bespoke Person-Centred Planning. Developed in partnership with the individual, and presented in accessible formats, each plan offers an outcomes-focused approach to develop and maintain skillsets, friendships and progression towards enhanced independence and choice.
